Community Support Program

 

 

Community Support is a community based service designed to assist individuals with serious and persistent mental illness to improve, maintain or restore stability in community living.

The services we provide to help consumer- survivors attain this end are listed below:

A community support worker counsels a consumer/survivor.
Services provided include:

• ongoing assessment of independent functioning, daily living skills, self-esteem, and interpersonal skills
• assistance in the development of realistic and attainable goals
• supportive counselling and assistance with problem solving and personal development
• teaching of independent living, functional skills, social skills and leisure skills
• coordinating services with other agencies, health care workers and significant others
• advocating with clients as necessary
• addressing issues related to symptoms, medication management and encouraging the follow-through of treatment plans
• providing crisis intervention, follow-up and outreach as needed

All support services are tailored to meet the unique needs of the individual, available for as long as the service is needed, provided in the environment of choice, and, are voluntary.

Community Support accepts referrals initiated by the family doctor, psychiatrist, community agencies, family members, and self-referrals.

 

Case Management

Designed to complement and enhance the Community Support Services offered by the Branch, Case Management provide individuals with the opportunity to participate in an intense, unique recovery process focusing on changing one’s attitudes, values, feeling, goals, skills and role in order to achieve greater wellness, happiness and life satisfaction. The service is client driven, structured, goal focused and time limited. Services are delivered through a shared team approach or through one to one support.

In addition to Community Support services listed above, Case Management offers:

• comprehensive assessments
• recovery self-care skills and strategies
• action planning
• intense case management
